Who decides coverage
Get the plan wording from your insurer in writing, and ask your provider for the billing code they will use. This page points to where coverage is decided.
Three sources, none on coverage
Two of the three sources come from one publisher, named in the first link. The third comes from a dermatologic surgeons' group. None is an insurance guide. Use them to prepare questions for the provider.
- The American Med Spa Association article on what to look for in a safe med spa visit covers what to check before you book.
- The ASDS medical spa safety resources overview collects safety resources for medical spas.
- The association FAQ collects common questions.

Five coverage steps
- Call your insurer and ask whether your plan covers Botox for your reason for treatment.
- Ask what documents the insurer needs.
- Ask your provider for the price if insurance does not pay.
- Ask whether the provider bills insurers at all.
- Write down the name of the person you spoke with and the date.
Get the answer in writing
A verbal "yes" from a front desk is not a coverage decision. Get the insurer's answer in writing. This site reads documents and is neither an insurer nor a clinic, so treat this page as a question list, not medical or coverage advice.
